TheCMSregularly inspects nursing homes to find out if the facilities deliver care that measures up to the CMS standards of care for the health and safety of the residents. The worst-performing nursing homes get put on the CMS Special Focus Facility (SFF) list.

Once on the list, the facility has to undergo more frequent onsite inspections until it either fixes its problems or CMS strips the facility of its Medicare and/or Medicaid certification. After a nursing home gets removed from the list, another facility will take its place on the list.
